From the reviews I have read the Nvidia DualTV and ATI Theater 650 have the best image quality. The Nvidia is a dual analog tuner, while the ATI has an analog tuner and a digital tuner (although I think only one can be used at a time). DualTV is around $140, while the ATI 650 is around $100.
3rd place is the ATI Theater 550, a single analog tuner
4th is the Hauppauge cards, PVR-150 single tuner and PVR-500 Dual tuner
There have been recent quality issues with the PVR-500. I don't know if they have been fixed or not.
I haven't read reviews for any other cards so you may want to research it or somebody who knows more about them can tell you. Other popular cards are the VistaView Saber 2020, it is a PCI-E dual tuner that uses the same chip as the Nvidia DualTV. Also Hauppauge just came out with the HVR-1600 which has an analog and digital tuner.

as for image quality the ati 550 is slightly better than the Hauppauge cards, but the ati software is terrible. Hauppauge has great software.
