like the title is it possible to use this card (http://www.newegg.com/app/ViewProductDesc.asp?description=14-131-236&depa=0) to run dual crt's and use the middle tv out to a 5 inch lcd?

hmmm, it has 3 outputs, so i guess they can all be used simultaneously (sp) i dont know how it would work out though

I don't think many 5" lcds run off of tv connections, you would have to build some sort of signal conversion. If the tv-out is analog (not sure) then it would be nearly impossible to make your own circuit unless you are very skilled. Digital to digital would be much easier.
Sorta. IIRC when there are two monitors hooked up, the video-out will only output a clone of one of the two monitors. Not very practical.

the 5 inch lcd i got is out of a psone screen and it has composite input already tested with my gforce 4 mx440 worked good thanks for the input and i was hoping that i could hook up 3 monitors to this card but probly not i guess i will see when it arrives...

It wont work. ATI's drivers/software will limit you to at most 2 displays on at one time on one card. If you had another PCI video card, then you could add more displays, but on a single card you are limited to 2 simultaneous monitors being displayed.

nah, most cards have a master head, and the other 2 are slave heads, and when 1 slave head is in use, the other cannot be used at all.
The only exception is matrox cards. they can support 3 and 4 monitors sometimes.
